Skin concern
Acne.
It's an inflammatory condition. Not a bacteria problem.
What's actually going on
Acne is driven by chronic inflammation, not bacteria. Bacteria play a role, but they're not the root cause. Acne can persist even after bacteria are eliminated. What's actually happening is an immune response that's lost its way. Chronic, low-grade inflammation sits in the background like a slow-burning bushfire. It targets vulnerable areas, and for many people that's the skin. Hormonal shifts increase sebum production, which activates inflammatory pathways. The immune system overreacts, tissue-resident memory T cells begin misfiring, and the cycle becomes self-sustaining. Breakouts aren't random. They're signals of a system that's stuck in a loop.
The common approach
The medical route is topical creams, antibiotics, or isotretinoin. These can suppress symptoms but often leave the underlying inflammation untouched. Antibiotics kill bacteria, but if bacteria aren't the root cause, the acne returns when you stop. Isotretinoin works but comes with significant side effects and doesn't suit everyone. The clinical route isn't much better. Standard peels and masks strip the barrier further, keeping the skin locked in an acidic pH that fuels the cycle. IPL and laser are marketed to "kill bacteria" but if bacteria aren't driving the condition, what is it actually doing? Both approaches share the same problem. They react to what's visible without addressing why it keeps coming back.
How we treat it
We work with the inflammatory process, not against it. Controlled needling acts as a reset. It triggers an acute inflammatory response, short-term and repair-focused, which redirects the immune system away from the chronic loop it's stuck in. During that window, targeted compounds are delivered into open microchannels to support barrier repair and modulate inflammation at the source. We needle active acne. Including pustules. The idea that you can't is a misconception based on an overly conservative approach that doesn't account for what's actually happening at a cellular level. If you understand the inflammatory process, you can work within it safely and effectively.
Treatments used
How they fit together
These aren't used in a fixed order and can be combined in a single session. Each treatment is chosen based on how your skin is presenting. Recurring breakouts and excess oil, we tap it. Deep congestion and cystic activity, we go alkaline and may tap the larger lesions. If the skin needs resurfacing, we peel or alkaline depending on the situation. Needling is the foundation because it resets the immune response and delivers compounds where they're needed. We don't escalate to peels or alkaline unless the skin calls for it. The priority is always to restore pH balance first, not strip it further.
